After Hours

When our office is closed, you can always speak to one of our physicians by calling 312-943-6964 and following the prompts to consult with a physician on call (option #4).

Parking

Parking is available via the covered, attached garage. The garage entrance is located on North Clark St. south of Dave and Busters. The parking rate is $6.00 for 3 hours with parking validation-which we will gladly provide for you! Once you park, you will take the elevators to the main lobby. Once in the main lobby, you will take the second set of elevators to the 4th floor. Metered street parking is available on Clark St. and nearby side streets.

Appointments

Child and Adolescent Health Associates Ltd. takes pride in offering a range of providers and appointment times to accommodate your busy schedule. Please call our office to make an appointment and our front desk staff will accommodate your desired time as best they can.  If you are running late or need to reschedule, please contact our office as soon as possible.

What to Expect at Your First Visit

  • You will be greeted by our front office staff and, depending on the type of visit, will be directed towards either our well or our sick patient waiting area.
  • Front desk staff will make a copy of your insurance card and obtain completed Patient Registration and Financial Policy forms from you.  They will also request that co-pays be paid at the time of visit.  Please note that CAHA only takes credit cards or checks but not cash.
  • You will be requested to complete a patient medical history questionnaire and present that to the nurse.

What to Expect After Your Visit

  • Most prescriptions will be sent electronically to your preferred pharmacy.
  • You may receive a separate bill from Quest Labs for any laboratory tests performed during your visit.
  • You may receive a patient statement from PediaTrust, our external billing company.  You can make paymentsby using our website, by calling PediaTrust to process your payment over the phone or by sending in your payment by mail.
  • Be sure to stay updated with our practice and pediatric information by pressing "Like" on our Child and Adolescent Health Associates' Facebook Page.

Insurance and Billing

We accept most major insurance plans. Unfortunately, none of the health plans offered on the Illinois Exchange in 2017 include Child and Adolescent Health Associates.
